1. The service

Duricor Health Monitoring lets you log health readings such as blood pressure, weight and symptoms on the website, in the app, on the phone line or by text message, and record your ECG (EKG) with your heart monitor and send it to your care team. Your care team can see your readings, recordings and reports, and may be alerted to readings, such as blood pressure, that need attention.

2. Not for emergencies, and not medical advice

Do not use the service for emergencies. If you have a medical emergency, call 911. Readings are not watched around the clock, and your care team may not see a reading right away.

The heart monitor records and sends your ECG; it does not diagnose heart conditions. The service does not replace your doctor. Reports, feedback and the health articles on this website are general information, not medical advice for your situation. Talk to your care team about your own care.

3. Who can use the service

The service is for adult patients whose care team has enrolled them.

4. Your account

  • Keep your password and your phone-line PIN private.
  • Enter your readings accurately; your care team relies on them.
  • Tell us or your care team straight away if you think someone else has used your account.

5. Text messages (SMS)

Duricor Health Monitoring sends text messages from (888) 705-2970. These terms apply to them.

  • What we send. Replies when you text us your health readings, reminders to log your readings, and occasional notices about the service, for example when our phone line is down. We do not send marketing or promotional messages.
  • Signing up. You receive our text messages only if you agreed to them: on your clinic’s enrollment form, by telling your care team, or by texting START to (888) 705-2970. Agreeing is not required to receive care.
  • How often. Message frequency varies. Most texts are replies to messages you send us; reminders and notices are occasional.
  • Cost. Message and data rates may apply, depending on your mobile plan. We do not charge for our text messages.
  • Stopping texts. Reply STOP to any message at any time. We will send one message to confirm you are unsubscribed, and then no more texts. To start receiving texts again, reply START.
  • Help. Reply HELP to any message, or email us at benjamin.tan@duricor.com.
  • Not for emergencies. If you have a medical emergency, call 911.
  • Phones and carriers. Our text messages work with mobile numbers in the United States. Carriers are not liable for delayed or undelivered messages.
  • Your privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. See the Privacy Policy for details.

6. The mobile app and your heart monitor

Use your heart monitor as your care team and the device instructions tell you. Keep the app up to date so your recordings reach your care team.

7. Using the service properly

Please do not:

  • use someone else’s account, or let someone else use yours without your permission;
  • try to get into parts of the service you are not meant to reach, or interfere with how it works;
  • use the service for anything unlawful.

8. Availability and changes to the service

We work to keep the service available, but it may sometimes be unavailable, for example during maintenance or an outage, and messages or readings may be delayed. We may change, add or remove features.

9. Website content

The articles and other content on this website are provided for your personal, non-commercial use. The website, app and software belong to Duricor or the people who license them to us.

10. Stopping use of the service

You can stop using the service at any time: reply STOP to stop text messages, and ask your care team if you want to leave the program. We may suspend access to protect the service or other patients, or if these terms are not followed.

11. Changes to these terms

We may update these terms and will show the new date at the top. If a change is significant, we will also tell you on this website or by message.
